The expectations the OP has seem perfectly reasonable to me. They want their boss to act to address the situation, and (correctly imo) recognise that it's their boss's job to determine what the appropriate action would be.

About midway down there is a bold heading Question, with two clear goals: "if the boss isn't aware of the difficulties: that he takes it seriously ... and eventually acts upon it" and "if the boss has known all along: that he discloses this fact to us..."
Mar 12 at 22:55 comment added TheDemonLord @PlayerOne - If you just want to vent to your boss - AKA Clear the Air - then just tell your boss - but reading the question, it's clear that the OP wants an outcome of sorts and so wants to approach it in such a manner to achieve that. However, without more detail about what that desired outcome is, we can't help.
Mar 12 at 22:54 comment added TheDemonLord @Zipper - So, I understand your point - but the problem is: How you approach this problem depends on what you want to happy and 'to just be happy again' isn't a good goal/outcome. If you want them to be fired (and there's nothing wrong with that) then the conversation is about how they aren't doing their job and the time/cost of having to retro-fix their work. If you want them to improve - then you approach by focusing on how to redirect behaviors into a productive avenue

There are too many unknowns in the equation to be talking about specific means. The answer to your question is: "To create an environment in which we can do our work again without being borderline sabotaged/demoralized." There are multiple ways how to achieve this and I'm fine with either one, really. But that's not the point of this Question. I want to get to ball rolling first - get the discussion started on the right foot and without being brushed off.

Explain the issues you had, use concrete examples identifying the outcomes he's caused (either problems or where the rest of the team had to cover) and ask what he thinks you should do. Focus on the problems and not the personalities
