I am doing night shift since 6 months, now I want to request my CTO and PM to end my night shift and to ask make my shift as morning.

What is a professional way to mail them regarding this? Also I need to mention few points like months I have been working, in the start I was told it will be for 3 months but now I worked for 6 months after many extensions, I cannot work more as I am facing health issues. So please suggest me the best professional way I can approach through mail.
